The Department of Health mandates that “each assisted living residence disclose certain information about the residence to each potential resident, the resident’s interested family and the resident’s agent as early as practical in the decision making process and at least prior to the admission decision being made.”- The Elms is a community that serves both independent residents as well as residents that may require some assistance with the activities of daily living. It is licensed by the Department of Health, State of Rhode Island.
- The Elms is owned by Mr. Guy Maiorano and operated by Mr. Robert E.P. Elmer III, the Certified Administrator of Record.
- The Elms is licensed as an M-1 community, which allows us to centrally store and administer medications, and also licensed as F-1 in our East Wing, which means our residents may defend in place in the event of a fire and F-2 in our West Wing, which means residents, must evacuate in the event of a fire or fire drill. All Elms residents must be capable of self-preservation in the event of emergency.
- Admission criteria require that the residents be able to ambulate on their own (mobility aids are permitted) and be able to transfer on their own. Incontinence is not a problem provided that the resident is able to manage it himself. Potential residents that require constant “skilled or nursing home care” are not eligible for residency.
- It is the policy of The Elms to discharge a resident for any of but not limited to the following reasons. 1. Failure to pay the basic service rate or additional charges for services that have been requested. 2. Failure to comply with State or local laws after having received a written notice of the violation. 3. Failure to comply with The Elms rules and regulations or if there is a change in the use of the community. 4. The Elms finds that the community is no longer appropriate to provide the level of care the resident requires.
- The Elms provides the following basic services 1. Three nutritionally, well balanced meals a day and snacks. 2. A comprehensive program of planned activities designed to meet the resident’s physical, spiritual or social needs. 3. Use of all common rooms. 4. Weekly housekeeping including linen change. 5. Laundry service (personal as well as flat laundry). 6. Individual heating and air conditioning systems. 7. Emergency pull cords. 8. 24-hour staffing. 9. Access to our staff R.N. 10. Transportation within the local area. 11. Cable Television.
- The cost of our private suites range from $3,395.00 to $4,095.00 depending on the configuration of the suite. A list of fees for services will be provided at the resident’s request. A security deposit that is equal to one month’s rent is required. The security deposit is returned when a resident leaves the community less any charges for damages. A 25% surcharged is assessed to the security fee to those residents that have pets. Smoking is not permitted anywhere in the community. Prior to admission, The Elms requires a one time non-refundable Community Fee of $2,300.00. The monthly fee is due on or before the first of every month. There is no lease and no “buy-in.” The resident or their responsible party is required to give 30 days notice of their intent to vacate the community. The resident or their responsible party will be notified 30 days in advance of any increase in rent or fees for services.
